
Practice Policies and Professional Guidelines
This section details the essential practice policies, fee structures, and ethical standards that govern Dr. Catherine M Johnson-Ford’s psychological services.
All listed credentials may be verified online, and are kept up-to-date.
Fees
$200 Initial Consultation
A comprehensive first session to assess your needs and goals.
$175 Family Therapy
Couples or other adult family members; covered by Medicare when an existing mental health issue is worsened by family issues.
Ongoing Individual Sessions
30 minutes $90, 45 minutes – $135, 60 minutes – $175
Regular sessions tailored to support your therapeutic progress. Appointments may be weekly or on a less frequent schedule, depending on your needs, logistics and preferences.
Group Therapy
$60 per 90 minute session held, billed by the month.
Letters & other documentation
Prorated charge based on my hourly rate of $175.

Policies, Credentials, Legal & Ethical
Privacy and Confidentiality
Your privacy is a top priority to me, and is insured by the law & ethics of my profession, as well as by HIPAA. All interactions with mental health professionals are private and confidential and will not be released without your written permission. For the vast majority of clients, no exceptions to confidentiality are made. However, the law requires therapists to make exceptions in certain circumstances such as reporting child abuse, or protecting human life.
Secure Technology
All of the technology I use, including Doxy and Zoom, is HIPAA-compliant. In the 15 years I have been using TherapyAppointment.com to manage my practice, there has never been a breach – there was one attempt but it was promptly thwarted.
Your rights regarding records
You have the right to review your own records and/or to have them sent to another professional. You also have the right to request letters or other documentation for third parties; I charge a fee (prorated from my hourly rate) for a letter. You may contact me directly to initiate a request (that will ultimately need to be in writing) by phone at (512) 842-2223, via TA portal secure message https://api.portal.therapyappointment.com/n/public/clientRegistration?therapistId=11eca5966b031cfdbdc00eb9c2fff873, or by mail at 538 Park Ave Ste 2, Orange Park FL 32073 to make your request in writing. Such requests will be fulfilled within 15 days of your request.
Payment Arrangements
Fees are due and payable at the end of each session, unless other arrangements have been made. Payments can be made via credit/debit card, check, or cash. Most people opt to keep their card on file using my secure portal; I will not have access to the card information itself. You may use an HSA or FSA card for therapy services.
Cancellation Policy
Please provide at least 24 hours’ notice to avoid a cancellation fee for missed appointments. I charge the amount I would have received had you been present. Such charges cannot be billed to insurance or HSA/FSA.
Refunds
Refunds are given only when there has been an error in a charge, or there is an unexpected difference in your favor between a copay/coinsurance/deductible paid and what your insurance actually covered.

I engage in ongoing professional development to stay current with best practices and developments in the field.

Credentials
I have been a practicing clinical psychologist for over 25 years. I’m licensed as a psychologist in both Florida (PY 12546) and Texas (25350). I am authorized by PsyPact (7646) to provide telehealth services in the 80% of states that participate (https://psypact.gov/page/psypactmap). I am QTAP certified, recognizing high standards of knowledge and care to LGBTQ+ people.
I obtained my Ph.D. from the University of Texas at Austin in 1994. I completed my B.A. in Psychology and Behavioral Sciences at Rice University in 1984.
